“ | Most creators of in-game universes are aiming for minimal ambiguity in their worldbuilding. Our understanding of our goals is fundamentally different! We aim for the opposite things. Ambiguity. Doubt. Subtext. Layers of meaning. The owls not being what they seem. Of course, sometimes a bullet’s just a bullet, and a barking dog is a dog that’s barking. However, the mass of ambiguous, fluid things is a critically important part of our setting. Does the outline of that chair resemble a guillotine? Great; our kind of chair. Is there something priestly about this butcher? Fantastic; let them scratch their heads. This doesn’t just apply to adding meaning to the simple things; the converse is also true— direct references and allegories should be reduced to small hints.

— Pathologic 2 art book |
The listed items are different types of miscellaneous items found throughout Pathologic 2 that can be used in barter, quests, medicine brewing, and specific trade with Var (organs) and the Fellow Traveller ("dead items").
Currency
Item | Description & Details | Use |
---|---|---|
Money |
Oddly enough, money is used to buy items in shops. Offering it during common street barter, however, would be seen as offensive. On the third day of the epidemic local prices went through the roof; now, on the eighth day, very few people agree to actually trade for money, preferring barter instead. The latter is more cumbersome, but can yield lucrative rewards. |
Can be used to buy items in Stores, Bad Grief's Nest, Andrey Stamatin's Broken Heart, and Var's Shop. |
Inquisitorial Coupon |
Food is only sold for these now.. The sword is mightier than the pen... unless it's an inquisitorial pen. |
Can be traded in stores for food after the arrival of Aglaya Lilich. |
Organs
Item | Description & Details | Use |
---|---|---|
Blood |
A blood-filled flask. Blood is one of the primal elements. It bears life and death. They say that a bloodless man is immortal. |
Can be sold to Var, the Fellow Traveller, or used in medicine brewing. |
Infected Brain |
A section of brain tissue. Crossed with defects. Rumor has it, sand pest can be transmitted through word and thought. Bullshit... it has to be bullshit. |
Can be sold to the Fellow Traveller, or used in medicine brewing. |
Infected Heart |
Ulcerous, yet still almost alive. "Every object has a heartbeat. Can you hear it?" |
Can be sold to the Fellow Traveller, or used in medicine brewing. |
Infected Kidney |
A sick human kidney. "Emshen" means "a doctor", or someone who knows the Lines. "Erdem" means "a wise man", who doesn't know them. Speaking in more modern terms, "erdem" would refer to a scientist. |
Can be sold to the Fellow Traveller, or used in medicine brewing. |
Infected Liver |
A sick human liver. A body is not to be cut open. It's only allowed for menkhu - wise men who know where a body wants to give - and butchers, who accept sin. The word for butcher, "yaragachin", also means "surgeon." |
Can be sold to the Fellow Traveller, or used in medicine brewing. |
Kidney |
A human kidney. According to the steppe beliefs, the body is a perfect representation of the Universe. However, the steppe people are not so naive as to correlate organs and bones to cosmic or geographical features. What they refer to are, first and foremost, connections and circulations. |
Can be sold to Var, the Fellow Traveller, or used in medicine brewing. |
Liver |
A human liver. Cutting open any bodies, including human, is taboo in our town. According to the "Baur Meges" steppe myth, the wholeness, harmony, and finiteness of the body are sacred. Only a person who ppossesses knowledge has the caste right to break this wholeness. Such people are allowed to work as doctors, butchers, or tattoo makers. |
Can be sold to Var, the Fellow Traveller, or used in medicine brewing. |
Heart |
A hot, almost-beating heart. In the steppe lore, the center of mind and knowledge is the liver, while the center of feeling and emotion is the lower guts. They view the heart as a less crucial and rather mechanical organ that simply pumps life around, oddly arriving at a consensus with science-based medicine. |
Can be sold to Var, the Fellow Traveller, or used in medicine brewing. |
Infected Blood |
Can be used like any other infected organ. Along the Layers of the body, Blood is often mentioned; at other times, Flesh; but rarely as two separate Layers. Blood is flesh. "We are of the same blood, you and I." "You are flesh of our flesh." |
Can be sold to the Fellow Traveller, or used in medicine brewing. |
Brain |
A section of brain tissue. "Blood is only a stream of red and white blood cells. Sure, they contain information, but how can anything compare to the magnificence of the brain?" Dankovsky once demanded. |
Can be sold to Var, the Fellow Traveller, or used in medicine brewing. |
Other
Item | Description & Details | Use |
---|---|---|
Beetle |
A surprising find in autumn, if not useless. Where do they go in winter? |
Can be traded with certain children. |
Bloody Bandage |
This is a dead item. Someone created it by using a bandage. According to sanitary procedures, any object that bears the trace of a deceased person's blood must be burned. According to local belief, it's pointless since the blood will simply appear elsewhere. |
Can be sold at the Dead Item Shop. |
Bracelet |
Worn to cut off the flow of warmth between the heart and the ring finger. Too small for me. A human body is the sum of its circulation. But it often needs to be interrupted for certain medical procedures. |
Can be used in bartering. |
Broken Ampoule |
This is a dead item. The carcass of an ampoule of morphine. Items used to improve health die. Who could possibly want this? |
Can be sold at the Dead Item Shop. |
Broken Scissors |
The Moerae must have been in a hurry. It's said that Clara the Changeling, a thief girl who claims to be a saint, can repair any sharp object just as easily as she cures the sand pest, simply by laying her hands on it. |
Can be used in bartering. |
Bull Bell |
Did it bring luck to the one who wore it? This bell's inner surface is marked with sigils from the secret, sacred steppe alphabet. Like a human body, its inside mirrors the universe. |
Can be used in bartering. |
Button |
A trifle to a human, but a whole eye to a doll. Murky is said to have a doll, but no one's ever seen it. Perhaps it simply lives all over the Town? |
Can be used in bartering. |
Candle Stub |
The stub of a nuptial candle. It smells of Earth. They say when Wild Nina Kaina, the Dark Mistress of our town, so much as glanced at candles, they'd bow to her–even without wind. |
Can be used in bartering. |
Chalk |
A piece of chalk. Used to draw lines. The world is one whole, having form only where it is divided. Some Lines are natural; others, man-made. Chalk, composed of the dead bodies of sea creatures, is a good tool for setting boundaries. |
Can be used in bartering. |
Charm |
Averts bad fortune. Unhelpful to me, but some people value it. You can't bring about good fortune--but you can craft it, like this handwoven charm. I suppose I could trade it for something. |
Can be used in bartering. |
Chisel |
Used to mark and sharpen tools. Handy when fixing bladed weapons and simple machinery. Rumor has it, Georgy Kain carved the statue in The Crucible's yard with his own hands. Drivel. Why would a Judge waste his time on that? |
Can be used to repair knives or scalpels, or used in bartering. |
"Dragonfly Eye" |
An incredibly rare concoction made of twyre berries. When consumed, it reveals several points of interest in the Town. Twyre berries are rarely picked. Few people ever see this potion. Perhaps fate is thanking me for lending someone assistance in the past? |
Can be consumed to mark points on the map, leading to events or boxes of items. |
Empty Bottle |
A clean bottle. Can be filled with water. No one drinks water from the Gorkhon river in our town, which is quite sensible. No one digs wells either, which is less so. Clear drinkable water is not a luxury, but is valued. It is brought from the steppe, and people can often tell which spring it is from by the taste. |
Can be filled with water to become Water Bottle, infected water to become Murky Water, or blood to become Blood. |
"Fingernail" |
The ferryman requires payment before he'll let me on the boat. This odd coin will do. Kids play "Mice and Guardians" with these. The rules of the game are unknown, but the rumors say they change every day. |
Can be given to the Boaters to travel across The Town. |
Fishing Hooks |
Sharp objects are regarded as dangerous and thus frowned on by the locals. Clara the deranged says that everyone has a hook to grab. But she's a manipulator. What everyone actually has is a knot to untie. |
Can be used in bartering. |
Grindstone |
Used to sharpen knives, scissors... and scalpels. Sharp objects are frowned upon, for they are capable of disrupting the sacred wholeness of the body. But you can't cut bread with tradition, so people end up breaking the rule. |
Can be used by townsfolk to repair Scalpels. |
Marbles |
Children give them names. They believe you can find a lost marble by calling after it. No one knows how marbles appear in the Town. No one makes or delivers them. They simply pop up here and there; on the floor, under the pillow, or right on the ground. |
Can be used in bartering. |
Match |
Can't light an oil lamp without one. One must recall that which is most dear to them before the match burns out, or the whole thing won't work. |
Can be used in bartering or used to light the Lantern. |
Metal Scrap |
Invaluable if one needs to fix a gun or a relatively complex contraption. Big Vlad's factories are one of the few stable workplaces in the Town. Unfortunately, he keenly remembers that 'stable' can have several meanings. |
Can be traded with Crows, Pochards, Swifts, and Tots and used to repair machines in the Haruspex's route. |
Needle |
As piercing as a wise person's stare. Can be used to pick small locks, such as the ones in cabinet drawers. They say that when Lara Ravel pricked her finger with a needle, no blood was drawn, and she hasn't ever fallen asleep since. |
Can be used to pick small locks or used in bartering. |
Package |
Contains unknown useful items. Unfold to extract them. Barter has a long history in our community. It has many styles, such as "tit for tat." Sometimes, people even blindly swap wrapped items. |
Can be opened to receive several random items. |
Pocket Watch |
Recently stopped. This means that the owner most likely died. Victor Kain never carries a watch around, but always knows the correct time, down to the second. We've tested him. Can he tell by the stars? |
Can be opened to receive several random items. |
Ring |
It circles back, like time. Equally precious. "Ding-a-ding, |
Can be used in bartering. |
Safety Pin |
Both safe and unsafe at the same time. Murky the orphan says that safety pins are like people: open or closed, they always have a pointy end. |
Can be used in bartering. |
Scrap Name |
When a person is done with their name, they discard it. People in this town are like a flight of birds, and birds don't require names. Individuality is overrated. |
Can be traded at the Dead Item Shop. |
Sewing Box |
Smells of home. Useful for intricate sewing and mending. Gravel once knitted a sweater for me. I asked her why it was blue. She told me that blue is the color of morons, and never made me another. |
Can be used for clothing repairs. |
Soap |
A bar of common soap. Nothing fancy, but soap is the best way to maintain hygiene. Essential. |
Can be used for bartering. |
Spindle |
It's almost out of thread. Victoria Olgimskaya used to spin. Only for show, of course. What's there to spin when your husband slaughters bulls instead of herding sheep? |
Can be used for bartering. |
Spring |
A rusty metal spring. Useful to fix all kinds of tools. Industrialization granted the local workers new rights and fixed working hours. All it asked in return was their free will. |
Can be used in bartering and used to repair machines in The Haruspex's route. |
Straight Razor |
Too wonky to be used as a weapon or surgical instrument, but kids love it. Ten years ago, local bandits used to call themselves "barbers" and brandish a particular style of razors as a mark of their identity. I wonder what became of that culture. |
Can be used to repair knives and scalpels or used in bartering. |
Thimble |
Useless in a town that forbids sharp objects. Still good for sewing. Young ladies of good upbringing are often taught embroidery. But young Capella never uses a thimble. A Mistress must sense a needle's path; if she doesn't, she deserves the consequences. |
Can be used by townsfolk to repair clothes. |
Thread |
Ties that which is torn; fates, if it's red. Serious surgery requires thread, but in the field there's often no time to sew together wounds. Bandages and tourniquets have to do. |
Can be used in bartering, repairing clothes, or upgrading inventory. |
Toolkit |
Used to make complicated repairs. According to Stakh, no self-respecting person would use such a kit; any decent master gathers their own collection of gears and scrap, instead of relying on premade solutions. Yet another thing we all do wrong, according to Stakh. |
Can be used to repair or upgrade the alembic in the Lair. |
Tweezers |
Useful to repair delicate equipment. Some kids sort their stamp collection with these; others tear wings off bugs. |
Can be used in bartering. |
Twyrine |
A herbal infusion. Drink to reveal places and events hidden from naked eyes. The town speaks. It calls to me. But it's hard to hear it over the rustle of my own thoughts. When they're shushed, though, one can sense the pulse of life all over the village. |
Can be consumed to mark points on the map, leading to events or boxes of items. |
The listed items are different types of miscellaneous items found throughout Pathologic: The Marble Nest that can be used in barter with Sticky or Shrew, as well as sidequests.
Item | Description & Details | Use |
---|---|---|
Beetle |
(1x1) A surprising, if useless, find in autumn. Some fifteen years ago, the Soul-and-a-Halves, the gang of children who are defined by sharing special bonds with their pets, actually considered to allow flower chafers as their “Halves” due to the beetles “looking as though they knew something”. The idea was quickly abandoned as ridiculous. |
Can be traded to Sticky or Shrew. |
Broken Ampoule |
(1x1) People would use any and all means possible to cure themselves of the disease. An ampoule that used to contain either life-saving or life-ending drug, served its purpose (whatever it was), and has now become completely useless. |
Can be traded to Sticky or Shrew. |
Broken Scissors |
(2x1) The Moerae must have been in a hurry--too many threads to cut these days. It is said that Clara the Changeling, a thief girl who claims to be a saint, can repair any sharp object just as easily as she cures the sand pest--by simply laying her hands on it. Perhaps this is the reason why people refuse to throw away useless junk like broken scissors. |
Can be traded to Sticky or Shrew. |
Candle Stub |
(2x1) The stub of a nuptial candle. It smells of earth. Eva Yan, the mistress of the Stillwater, often burned candles in the study during the night, which gave her observatory-like mansion a cozier appearance. After Eva's tragic suicide, candles can no longer be lit in the Stillwater and go out immediately. |
Can be traded to Sticky or Shrew. |
Fishing Hooks |
(1x1) Sharp objects are regarded as dangerous and thus frowned upon by the locals. While the Gorkhon river has plenty of fish, fishing is for some reason unpopular among the locals. Perhaps it has to do with the fact that hooks are sharp objects, which are heavily frowned upon around here. |
Can be traded to Sticky or Shrew. |
Lockpick |
(2x1)A tool to open ordinary doors. Children make them from random trash. These lockpicks are better than the kind the locals would stick into each other's' guts, before the second decree of the late governor Saburov. |
Used to unlock doors. |
Marbles |
(1x1) Children give them names, as though they were alive, and believe them to hold memories. No one knows how exactly marbles appear in the town. No one delivers or makes them. They simply pop up here and there; on the floor, under the pillow, or right on the ground. |
Can be traded to Sticky or Shrew. |
Match |
(1x1) Draw the short one! One must remember that which is most dear to them before the match burns out, or else the whole thing won’t work. |
Can be traded to Sticky or Shrew. |
Money |
(1x1) Oddly enough, money is used to buy items in shops. Offering it during common street barter, however, would be seen as offensive. On the third day of the epidemic local prices went through the roof; now, on the eighth day, very few people agree to actually trade for money, preferring barter instead. The latter is more cumbersome, but can yield lucrative rewards. |
Can be exchanged for food the reopened shop. |
Package |
(2x2) The steppe people bury the remains of their dead in small packages. This is hardly one of them, but you’d rather not check. Long before the current socioeconomic catastrophe, the local townsfolk developed a tradition of barter that the whole community revelled in. Including many types, such as “tit for tat” and “retrab”, it is sometimes secret, with people swapping wrapped items blindly. |
Can opened to reveal a random assortment of items. |
Pocket Watch |
(2x1) Recently stopped. This means that the original owner has most likely died. Time doesn't seem to be working properly in the town; there's no other explanation to the fact that Georgiy Kain, the Judge, is two times younger than his twin brother. The Kains' fascination with all things clockwork must have something to do with this. |
Can be given to a Steppe man outside of Aspity's residence in exchange for his dream. |
Spindle |
(2x2) It has almost run out of thread. One can hardly jab a finger with this thing on accident. However, a hand can probably be punctured through by it, if enough pressure is applied. |
Can be traded to Sticky or Shrew. |
Used Bandage |
(1x2) The traces of the epidemic are omnipresent. According to the sanitary procedures, any object that bears the traces of a deceased person’s blood must be burned. According to the local belief, it’s pointless since the blood would simply appear elsewhere. |
Can be traded to Sticky or Shrew. |
The fifth of five tabs in the players inventory is "Other." Some items in this tab may be traded, sold, or used in quests.
Herbs and Ingredients
Item | Description |
---|---|
Black Twyre Herb |
A rare herb with unique properties. It is popularly called "woegrass". |
Bloody Twyre Herb |
One of the more popular types of twyre. An essential component of the famous twyre infusions, which vary based on the age and amount of the herb. |
Brown Twyre Herb |
One of the most fearsome types of twyre. The Steppe people believe that this herb is sentient and hostile to humans. The elders call it "the herb of Worms". |
Swevery Herb |
A rare herb. The people of the Steppe don't worship it as much as twyre, but they use it in their potions, because it greatly amplifies the medicinal qualities of twyre. |
White Whip Herb |
A rare valuable herb. The people of the Steppe don't worship it as much as twyre, but they use it in their potions, because it diminishes the danger of plucked twyre. |
Blood |
Human blood. Extracting organs from a human body is seen as a horrible blasphemy in the local tribal community. Only the menkhu are allowed to do this because they have an ancestral right to cut living bodies. However, some gatherers and also the Hunchback from the Atrium district would be willing to trade for something this rare... |
Infected Blood |
Menkhu know how to prepare healing potions by treating blood with twyrine infusions. In this blood, harmful microorganisms are probably winning a fight against antibodies. The blood's composition is constantly changing. A lot depends on the stage of infection. |
Heart |
A human heart. Extracting organs from a human body is seen as a horrible blasphemy in the local tribal community. Only the menkhu are allowed to do this because they have an ancestral right to cut living bodies. However, some gatherers and also the Hunchback from the Atrium district would be willing to trade for something this rare... |

Other
Item | Description |
---|---|
Bone Necklace |
A beautiful necklace carved from a bone that does not resemble bull horn, but rather mammoth or elephant tusk. Smooth and heavy. A prominent example of primitive art. |
Bronze Bracelet |
A very simple, old bronze bracelet. It may be hundreds of years old. |
Brown Rat |
A live rat with brown fur. Looks quite agile. |
Chest Badge |
An emblem of a bull's head; such badges are worn by the local police forces. Taken from a dead patrolman. |
Combustible Fluid |
A bottle of combustible fluid. Clear evidence of the demise of one of the terrible firestarting zealots, dubbed Exterminators by the people. |
Crowbar |
Looters use tools like this to break into bolted houses. |
Earrings |
Beautiful antique piece. Earrings with ornaments of this kind are sometimes found in southeastern burial mounds. |
Empty Bottle |
Can be filled with spring water. The water is in hide demand among the townsfolk, especially the drunkards, because it is good for quenching thirst. It's thought that water in the drinking fountains and water pumps comes from the town springs, while water barrels carry water from the spring of Sugagh Khadugh. |
Flower |
Women like flowers--both little girls and lascivious dancers. |
Gold Ring |
A beautiful gold ring. Such rings used to be in fashion some thirty years ago. |
Hook |
An ordinary fishing-hook. Can be of significant value to children, and not only because it's sharp. A hook is a somewhat grim element of local children's superstitions, connected with burial rituals. |
Kerosene |
Kerosene is used for refilling the lamp. |
Lens |
Lenses like this can be set into an optical apparatus and used to identify dense clusters of infected particles. Different lens sets allow for early detection and identification of different types of infected clusters. |
Needle |
A large sewing needle. Such needles may be of interest to children because local tradition prohibits them from owning any sharp objects. |
Rat |
A living grey rat. Not very large, but nimble. |
Razor |
A straight razor. A fashionable trinket among the teenagers who like to show off to each other with an item of dubious standing in local morals. |
Razors |
No crime is greater in this community than violating the integrity of a body with a sharp piece of metal. |
Silver Ring |
An old silver ring. Crafted somewhat roughly, the ring still looks quite valuable due to its age. Rings with ornaments of this kind are sometimes found in southern burial mounds. |
Watch |
There is nothing special about it, although the case and lid seem to be made of lowgrade silver. |
